Activity Sign-Ups

To participate in a high school activity, a student must pay the appropriate fee and have on file in the high school activities office, the following:

1. MSHSL Eligibility Brochure signed by a parent and the student  .......   ONE PER YEAR (This is now a combined Athletic and Fine Arts Eligibility Form along with Concussion Management Recommendations)
   
2. MSHSL Sports Qualifying Physical Clearance Exam Form   .......   GOOD FOR 3 YEARS
3. FLHS Emergency Contact & Insurance Information Form signed by parent/guardian   .......   ONE PER SPORT
4. Fairview Athletic Trainer Consent to Treatment Form authorizing treatment, signed by parent   .......   ONE PER YEAR
5. Fees (listed on coach & fee tab) and $100 deposit (returned from the coach at the end of the season, after equipment and uniforms are returned)   .......   Pay on-line available

Print out forms and fill out completely.

Bring all forms into the Activities Office prior to the start of the season, do not give forms and fees to coaches.  Athletes will be given a clearance card once all paperwork, activity fees, and deposit are turned in.  The clearance card must be given to the coach on the first day of practice, athletes will not be allowed to participate or try out with out one.
