Enroll in our schools

To Register at Forest Lake Area High School: Print & fill out the forms at the right, & turn them in to our Student Services Office.  Forms 1, 2, 3 & 4 are all needed to register.

Additionally, the following information is needed:

• Previous school’s address, phone number and fax number.

• A copy of the student’s immunization record.

• The family’s head of household information & previous address.
